[Cancer Type] has to be in the detail of Filter sheet for the action to get applied to image sheet as they come from two different data sources.

If I add the [Cancer Type] into the detail of the Filter sheet I will have one shape for each cancer type. That is not what I want. Do you have any other option?
As far as I know there isn't a straight forward option. As 'Gender and Race' is from the secondary data source,Tableau is mapping all Cancer types in Primary to all Gender and race in secondary. So when action filter is targeting the body sheet, it isn't filtering on Gender and Race as according to Tableau every cancer type in body data set has all the gender and race values.
I re-created the Image data set, with taking all the possible [Gender and Race] and [Cancer Type] combinations and applying appropriate X and Y coordinates you have. As the [Cancer Type] isn't changing by year for a gender/race, I don't have to consider Year in the image data set.
Now my new data set has, Cancer Type, GenderandRace, X and Y coordinates. Now, blending on Cancer Type, [Gender and Race] with two data sets, filters the body sheet accordingly.
I have attached a very rough sheet of the work I have done, format it if you believe everything is working as expected.
