The only way I've ever done something similar is to create a fake table with an action filter for each "row" of the table.
An action filter cannot be used at the individual row level for a multi-row table. So the trick is to create a one-row table for each row in the real table. In your case, that means 5 separate one-row worksheets. Then you can create a single action filter per row.
Put the rows in a vertical container and you have something that LOOKS like a single 5-row table but isn't. Then your hyperlinks will work as you want.
