I may not have caught the gist,
but I think it may be more tractable as a CASE Statement:
CASE [Current abilities (prior week)]
WHEN "Activity not attempted" THEN 1
WHEN "Unable to do" THEN 2
WHEN "with MUCH difficulty" THEN 3
WHEN "with SOME difficulty" THEN 4
WHEN "Without ANY Difficulty" THEN 5
Hmmm...this looks plausible. The problem has been recognition of the different levels. I’ll give it a try.
