It is likely you are referring to the warning message below. In which the only way to get around the warning is to remove the index() or rank function. This causes you to have the total column for all countries irrespective of the top 15 filter on the index.
You can get around this by creating the below calculated field with the LOD calculation.

I think you are referring to the fact that tableau shows grand total taking all the data into account and not TOP 15.
In general you've got 2 options:
1. If there is only 1 dimension you want to rank:
2. In case of more dimensions the only way around that i am aware of would be to use much more complicated table calculations:
But I think you're lucky and the 1st option is what you want, i.e. simply filter top 15 by number of records descending.
