It can be set up on any machine you like, with some requirements (see the install_guide). You'll want to run it on a machine that is always running, and ideally being monitored so that you are notified if it goes down. It must run against Tableau Server, but does not need to run on the same machine(s) as Tableau Server. You can't run it against Desktop, no. Yes, you will need your Server admin to do most, if not all, of the setup of VizAlerts. It uses trusted tickets, which requires that Tableau Server must whitelist the IP address of the machine you're using to run VizAlerts--so as long as your remotely hosted server can do that, I don't see why it should be a problem. Easier to run it directly on the Tableau Server machine itself, in that configuration, but you don't have to do it that way.
