Is there custom SQL being used in the connection?
Select vc.[_Case Master ID], vc.[_Caseload Assignment Current],
vc.[_Participant Name], vc.[Case Status], vc.[Reporting Structure Level 4],
vc.[Reporting Structure Level 5], vc.[Application Date], vc.[Eligibility
Date], vc.[Eligibility Extended To Date], vc.[Days in Application],
From biVR_Cases vc
LEFT outer JOIN buCase_Note cn ON vc.[_Case Master ID] = cn.Case_Master_ID
and cn.[Case_Note_Summary_Narr] like 'Ineligibility%'

The error is likely to do with the custom sql. I'm not an expert on writing sql, however the issue might be using the '=' on the left outer join. If more than one record is pulled using the '=' may result in the error.
Found more information on the MS SQL error here: dBforums Everything on Databases, Design, Developers and Administrators
Thank you all for the responses. I found a single duplicate record in the database that was causing all the errors.
Ahh wow, I'm glad you found the issue!
I hope you didn't have to dig through too many rows. Thank you for sharing, it may help others down the road.
