The criticality dimension will break the ranking calculation.
The right steps for avoiding that is:
1.Create a set on Med Center with top 5 as condition
2.Drag the set to filter
3.Drag criticality to color shelf
These kinds of issues are difficult to diagnose from screenshots alone. (For example, we cannot see the Marks count at the bottom of yours.)
As a first guess, why don't you use RANK_DENSE( ) in place of INDEX() as that will alleviate the tie for 5th place.
Meanwhile, if adding the Criticality field to the Colors card changed the number of bars, then it had to have retrieved more Records even though you appear to have a great many filters applied.
If you can provide a sample workbook, the community can make quick work of assisting you. Otherwise, it's not entirely obvious what's "wrong" with your second screenshot versus the first.

Although I am not selecting top N results, whenever I am adding criticality to colors it is changing the quantitative values associated with it. meaning for A suppose total count is 4 then adding criticality in colors changing the total to 4.6 or something.
Can you please let me know the solution for this? Now this is way beyond than Top N.
