Multiple date fields in single line graph

I have a dataset with multiple dates fields associated with each record - eg Date received, Date accepted, Date published, etc.

It looks something like:

I would like to show a count of distinct records for each field using the same x-axis - that is

three separate lines on the same graph:

top line shows number of distinct records received in July 2014

second line shows number of distinct records accepted in July 2014

third line shows number of distinct records published in July 2014

The result would look something like this but note this uses a single date field to generate the axis and then adds multiple fields to the chart.
