You'll need to do some additional data prep. Can you provide a sample of of your data? If so, we can probably show you how to do this in Tableau Prep.
This is tricky data to deal with. I don't think you'll be able to do what you need within Tableau Desktop, but you can do it in Tableau Prep. If that's an option for you, here's how:
Start by connecting to your data.
Add a new clean step, then do a custom split with the following options:
That will split it into multiple columns with individual values.
Now create a new pivot step and drag all the split fields into the "Pivot Values" section.
You'll now have a column with each individual medication.
At this point, you can probably just remove Pivot1 Names and Meds (the original field).
You'll have some blanks/nulls, so you may want to filter those out.
Finally, you may have some duplicates, so you might want to use an aggregate step to remove those.
Then you can write it to an output.
