How do I create a set that combines all discrete fields on row shelf?

Hi.  I am trying to follow the steps here: Sometimes You Have to Break the Rules. Expanding Beyond 16 Columns without Header Concatenation | Tableau Software to setup a report that displays more than 16 columns.  I am getting hung up on creating a set made up of all of the discrete fields in the Row Shelf.  Can someone please explain how I can do this?  I've tried selecting them all on the shelf and right clicking.  I've also tried multi-selecting in the Data window and right clicking, as well as trying to select the data displayed in the table and right clicking.  When I setup the set from the grid though, I only end up with a single row in my set.  Any guidance here would be greatly appreciated.  Thank you.
